Why does a new building still need door work?

Three reasons, and they show up in the first two years. A door specified on a standard cycle rating gets installed in a building that turns out to run three shifts. Safety devices are commissioned once and then never tested again, although the industry asks for a monthly check. And the operator is sized for the door rather than for the number of times a day the tenant actually opens it. None of those is a defect in the door; all three are a specification that stopped matching the building.
